Karmic Scale is a relic item in UnderMine. It costs 5 Minor Curses at Sho'guul's lair.
It sets the player's maximum health to 5. It cannot be overwritten by any health-altering Blessings, Curses, or Relics. Every source of damage and healing will inflict and heal 1 point respectively.

- 108 Beads: Heals the player for 20% health when praying at an altar.
- Aphotic Charm: Heals the player for 20% health if they have any curses each room.
- Blood Bomb: Heals the player for 20% health for every enemy killed with a bomb.
- Catalyst: Doubles all healing.
- Glass Cannon: Gives its damage with no health decrease.
- Gold Tooth: Makes each gold nugget heal 20% health.
- Hungry Ghost: Makes each enemy killed heal 20% health.
- Meal Ticket: Food now heals 20% health.
- Totem of Life: Makes each room entered heal 20% health.
- Wet Blanket: Prevents lethal damage 3 times.
- Poison Mushroom: No longer removes Max Health.
Negative[]
- Aegis never gives the player increased defense.
- Archer's Pendant, Axe Thrower's Pendant, Berserker's Pendant, and Knight's Pendant: Never give any extra damage.
- Pocket Grill just raises food prices e.g. in shop, but does not change healing effectiveness.
- Hot Cross Bun, Iron Branch, Mediocre Ring, Mushroom, and Ursine Ring: Don't give the player any extra maximum health.
- Soul Guard: becomes useless.
- Poison or fire damage will drain all health squares over a few seconds, effectively killing you as soon as you are affected.
- Momentary Peril still brings you down to 1 HP.
- Elixir only heals 1 HP.
- Numbing Cream becomes useless.
- Gordon's Tunic becomes useless.
